Volunteer With MOMS

As a MOMS volunteer, I understand that I will not be reimbursed for any mileage or out of pocket expenses, or for any injury incurred by me while rendering my voluntary services. I also understand that I am not covered by Workers Compensation Insurance and hereby waive and release the above organization in regards to any such claim which may arise.

I give MOMS permission to add me to their electronic newsletter distribution list from which I can unsubscribe at any time.

I authorize use of any photographs/video taken of me and/or my family member, and information I have shared, for advertising, educational, fundraising and/or promotional purposes. I understand that I will not receive compensation in exchange for the use of the photographs/ video/material and hereby release MOMS from any and all claims which arise out of or are in any way connected with such use.

I hereby certify through this electronic signature that all statements made on this consent form are true and correct to the best of my knowledge.
